& Part B) through Social Security.

You can enroll in one of three ways:

✔ By phone at 1-800-772-1213
✔ In person at your local Social Security office

EXTRA HELP (LIS) & MEDICAID

Extra Help, also called the Low-Income Subsidy (LIS),

is a program from Medicare that helps people with

limited income and resources pay for their prescription

drug costs.

If you qualify, Extra Help can:

Lower your monthly Part D premium

Reduce or eliminate your annual deductible

Significantly lower copays for your medications

Protect you from the Donut Hole (coverage gap)

Some people are automatically enrolled (for example,

those with Medicaid or receiving Supplemental Security Income), but you can also apply directly if you think you

may qualify.

Do I Qualify?
You may qualify if your monthly income and savings fall below certain limits. These limits can change yearly,

and help is available even if you don’t qualify for

Medicaid.
